Authorization and Release Form

The conference is a great opportunity for the EAST Initiative to document the experience of students and other EAST participants. We do, however want to ensure that all EAST participants are protected and aware of the possible use of their images. All conference participants are asked to complete a Name, Appearance & Image Authorization & Release Form.

You can also find this form on your “My Planner” page. Please download and print the release form for all registered facilitators, chaperones, and EAST students attending the event. Forms must be signed and a JPEG version uploaded by January 27, 2010. This is also the student registration deadline.

Once a signed copy of this form is uploaded with your registration, it can be used each year. This means that next year you will not have to repeat the process.

For a last-minute attendee, please bring completed authorization and release form to the Conference and turn it in at registration.

Chaperone Forms

Note: Chaperones (registered conference attendees not having an EAST login) should bring their signed form to the Conference and turn it in at registration.

Can’t be Photographed or Captured on Video?

If for any reason a parent prefers that their child not be filmed or photographed, the student will not upload a form. Instead, he or she will be unobtrusively identified at conference so they will not be included in any publications.
